From the 14th of May 2024 we will be piloting SystmConnect, a tool for submitting online requests to the Practice. 

In the initial pilot stage, we will be offering  patient's the option to submit requests for Fit notes (Med3's). 

Following successful completion of the pilot, we will look to expand the service further to other request types. 

How do I use SystmConnect?
Click the button below to be directed to the SystmConnect page. Simply fill out the questionnaire and submit.

Do I have to use SystmConnect?
No, SystmConnect is completely optional and is intended as an alternative option for submitting certain requests to the Practice. You can still contact the surgery by telephone to make these requests if you are unable to submit an online form. 

Three Chequers Medical Practice would like to invite you to use the Airmid App to book appointments, request medication and access your medical record.

The Airmid app can be used to:

• View your consultation notes, medical history and vaccination history
• View and add allergy/drug sensitivities
• View correspondence e.g. Referral/Discharge letters
• See communications between your GP surgery and other services, such as Hospitals
• View test results e.g. Pathology/Radiology reports
• Request and manage appointments and prescriptions

Three Chequers Medical Practice would like to invite you to use the NHS App.

Try the NHS App. If you’re a patient at our practice you can now use the new NHS App, a simple and secure way to access a range of NHS services on your smartphone or tablet. You can use the NHS App to get health advice, order repeat prescriptions, manage your hospital referrals, view your GP health record and more. If you already use SystmOnline and/or Airmid, you can continue to use these but you can use the NHS App as well.
