 SystmConnect is an online platform for submitting clinical and admin requests to the Practice. 

Patients are able to submit the following online requests to the Practice using SystmConnect:

1.  Submit requests for new or ongoing medical problems. 
2. Request for a Fit Note (Med 3 ).
3.Submit a travel clinic request.
4. Update your contact details/address.
5. Request Non-NHS work (Forms/Reports/Letters).

The Practice Team continues to work on developing new request types to add to the SystmConnect platform.

How do I use SystmConnect?
Click the button below to be directed to the SystmConnect page.  Choose the questionnaire you require, complete and submit.
You do not need to have an account to use SystmConnect but you must be a patient registered with our Practice.

SystmOnline

SystmOnline in an online service which allows you to book appointments, order repeat prescriptions and view your medical records.
This service is open 24/7 and be can accessed from your home PC, tablet or mobile phone.

Three Chequers Medical Practice would like to invite you to use the Airmid App to book appointments, request medication and access your medical record.

The Airmid app can be used to:

  • View your consultation notes, medical history and vaccination history
  • View and add allergy/drug sensitivities
  • View correspondence e.g. Referral/Discharge letters
  • See communications between your GP surgery and other services, such as Hospitals
  • View test results e.g. Pathology/Radiology reports
  • Request and manage appointments and prescriptions

NHS App

Three Chequers Medical Practice would like to invite you to use the NHS App.

Try the NHS App. If you’re a patient at our practice you can now use the new NHS App, a simple and secure way to access a range of NHS services on your smartphone or tablet. You can use the NHS App to get health advice, order repeat prescriptions, manage your hospital referrals, view your GP health record and more. If you already use SystmOnline and/or Airmid, you can continue to use these but you can use the NHS App as well.
