Three Chequers Medical Practice update
PLEASE note all of our surgeries remain closed to walk in patients – entry will not be permitted unless you have been offered an appointment with a clinician .

Anyone attending the surgery must now wear a covering over their nose and mouth; whether it be to attend an appointment, collect a prescription, or accompany someone they care for.
Endless Street Surgery:
Our Endless street Surgery has a specific collection point for pre ordered medication and paperwork only.
- Endless Street is not open for face to face appointments, however we do offer telephone appointments. Endless street is the centre of our telephone system, to book an appointment or general enquiry please call the surgery on 01722 336 441.
Three Swans Surgery :
Three Swans remains open for pre booked appointments only.
Please do not arrive early for your appointment - in accordance with current social distancing measures we are not permitting patients to wait inside the building , please therefore be mindful of the weather conditions when setting out and come prepared .
Three Swans Surgery Hot Site:
The rear ground floor is by appointment only for patients with symptoms which could be because of Covid19.
Winterslow :
We are pleased to announce that we have re-opened our Winterlsow Surgery for the collection of medication only - Monday to Friday 8am – 12.30pm.
- Winterslow is not open for booking appointments, ordering repeat prescriptions or general enquires .
Porton:
Porton surgery is open for pre booked appointments only and the collection of medication.
- Please do not go to this surgery to book an appointment, order repeat prescriptions or general enquires
Repeat prescriptions can be order by dropping a repeat request slip at any of the surgeries, by emailing prescriptions.3chequers@nhs.net or registering with systemonline
