NHS England - Confirm your home address

NHS England will be running a pilot from December 2025 until February 2026 that will allow patients to update their address digitally.

A small sample of patients will be contacted because NHS England hold a valid email address for them in the Patient Demographics Service (PDS). Patients will be asked to confirm if they:

  • still live at the registered address we have for them
  • have moved to a new address in England
  • have left the UK, either permanently or temporarily

As soon as patients have confirmed their address or that they have moved, their record will be updated in PDS. Patient's will  receive an email confirmation of any changes that have been made.

If a patient does not have the highest level of identity verification in the NHS App, they will need to call the NHS England Customer Contact Centre (CCC) on 0300 311 22 33 to confirm their address.

If a patient's address has changed and they are within the catchment area then the Practice will update your address on the patient record. If the address has changed and they are no longer in the area, then patient's will need to register at an alternative surgery using the Find a GP service.

NHS England will contact patients through the NHS App in the first instance. If patient's do not interact with the App within 3 days they will be sent an email. If they do not respond to the email address, they will receive an SMS text message within 8 days. If patient's do not respond to any of the previous communications within 28 days of getting the App message, NHS England will send a letter to the address NHS England hold for a patient.
